About the Role
Bluestones Medical is currently recruiting an experienced Band 8b Pharmacist to work within an acute hospital setting in Liverpool.
This is an excellent opportunity for a highly experienced Pharmacist to join a busy clinical environment on a temporary contract, commencing ASAP and running until mid-September.
The successful candidate will provide expert clinical pharmacy services, supporting the safe and effective use of medicines across the acute hospital setting. You will work closely with medical, nursing and multidisciplinary teams to optimise patient care and ensure the highest standards of medication safety.
Key Responsibilities
- Provide a high-level clinical pharmacy service within an acute hospital environment.
- Undertake clinical medication reviews and optimise medicines for individual patients.
- Provide specialist pharmaceutical advice to medical, nursing and multidisciplinary teams.
- Review prescriptions for clinical appropriateness, safety, interactions, allergies and dosage.
- Support the safe and effective use of medicines throughout the patient journey.
- Advise on complex medication-related queries and clinical situations.
- Contribute to the development and implementation of medicines optimisation initiatives.
- Support the safe management of high-risk medicines.
- Provide advice on antimicrobial prescribing and stewardship where appropriate.
- Support medication reconciliation and safe transfer of care.
- Participate in clinical governance, audit and quality improvement activities.
- Contribute to the development and review of pharmacy policies, procedures and guidelines.
- Provide professional leadership and support to junior pharmacy staff and other members of the wider healthcare team.
- Support the training, education and development of pharmacy staff and other healthcare professionals.
- Work collaboratively with consultants, doctors, nurses, AHPs and other members of the multidisciplinary team.
- Ensure all pharmacy practice complies with relevant legislation, professional standards and hospital policies.
- Promote patient safety and continuous improvement across the pharmacy service.
Essential Requirements
- Current registration with the General Pharmaceutical Council (GPhC).
- Extensive post-registration experience as a Pharmacist.
- Experience working within an acute hospital or secondary care environment.
- Previous experience working at Band 8a/8b level or equivalent.
- Strong clinical pharmacy knowledge and experience.
- Excellent understanding of medicines optimisation and medication safety.
- Ability to manage complex clinical and pharmaceutical situations.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Strong leadership and decision-making skills.
- Commitment to high-quality, patient-centred care.
