We are currently supporting a Central London primary care provider who is exploring the addition of a locum Band 7 Diabetes Specialist Nurse to enhance their long-term condition management services.
This is an exciting opportunity to work within a forward-thinking, patient-focused environment, supporting diabetes care delivery across GP practices and wider PCN services.
Key Responsibilities:
- Deliver specialist diabetes clinics (Type 1, Type 2, and pre-diabetes)
- Provide insulin initiation and titration
- Conduct structured diabetes reviews and care planning
- Support complex patient management and reduce hospital admissions
- Deliver patient education and self-management support
- Work collaboratively with GPs, Practice Nurses, and MDT teams
- Contribute to QOF targets and long-term condition frameworks
- Provide clinical advice and guidance to wider practice teams
Requirements:
- Registered Nurse with valid NMC registration
- Proven experience working as a Diabetes Specialist Nurse (Band 7 level)
- Strong background in primary care, community, or outpatient diabetes services
- Competent in:
- Insulin initiation & adjustment
- Diabetes education programmes
- Managing complex diabetic patients
- Experience using EMIS / SystmOne (desirable)
- Ability to work autonomously and within MDT environments
Desirable:
- Postgraduate qualification in Diabetes Care
- Independent Prescriber (highly desirable but not essential)
