About the Role
Bluestones Medical is currently seeking compassionate and experienced Community Nurses to join our agency team supporting services across Betsi Cadwaladr University Health Board, with placements primarily in Deeside and Chirk. This is an excellent opportunity for nurses who enjoy autonomous working and delivering high-quality care within patients’ homes and community settings.
As a Community Nurse, you will provide holistic, patient-centred care to individuals with a wide range of health needs, supporting them to remain independent while receiving treatment outside of hospital.
Key Responsibilities
Deliver high-quality nursing care in patients’ homes and community clinics
Assess, plan, implement, and evaluate individualised care plans
Administer medications, injections, and wound care treatments
Manage long-term conditions and support rehabilitation
Monitor patient progress and respond appropriately to clinical changes
Provide health education and support to patients and families
Work independently while liaising with GPs and multidisciplinary teams
Maintain accurate and timely clinical documentation
Adhere to safeguarding, infection control, and community care policies
Requirements
Valid NMC registration as a Registered Nurse (Adult)
Minimum 6 months recent UK community nursing experience (preferred)
Right to work in the UK
Full UK driving licence and access to a vehicle (essential)
Strong clinical assessment and organisational skills
Ability to work independently and manage caseloads
Excellent communication and interpersonal skills
Commitment to delivering high-quality patient care
Pay Rates
💷 Competitive hourly rates from £17.00 to £34.00 per hour, depending on shift type and experience.
