Senior Community Mental Health Nurse (Band 6)

Location: Hall Court, Telford
Hours: 37.5 hours per week (Full Time)

About the Role

We are seeking a motivated and experienced Senior Community Mental Health Nurse (Band 6) to join our dynamic Adult Community Mental Health Service in Telford and Wrekin.

This is an exciting opportunity to play a key role in delivering high-quality, person-centred care to individuals experiencing mental health difficulties. You will manage a complex caseload, contribute to service development, and support colleagues through supervision and clinical leadership.


Reporting Structure

  • Reports to: Operational Lead / Quality Lead

  • Accountable to: Service Manager / Quality and Governance Lead


Key Responsibilities

  • Manage a complex caseload, including assessment, care planning, and evaluation of treatment plans

  • Undertake comprehensive clinical assessments to identify needs, risks, strengths, and goals

  • Develop and implement formulation-based care plans in collaboration with service users and carers

  • Deliver evidence-based nursing interventions aligned with agreed care plans

  • Provide clinical supervision and support to team members where appropriate

  • Participate in quality assurance activities, including audit, monitoring, and service improvement

  • Contribute to the development of clinical standards in line with current guidance and policy

  • Work flexibly across service functions based on skill set and service needs

  • Service users, carers, and relatives

  • Local authorities and police

  • Primary care professionals

  • Charitable and voluntary organisations

  • Other healthcare providers

  • Employment and education services


Key Duties

  • Deliver the full nursing process, including assessment, planning, implementation, and evaluation

  • Provide consultation, advice, and clinical supervision to colleagues across the service and wider system

  • Maintain a strong focus on recovery-oriented, person-centred care

  • Ensure all interventions are delivered in accordance with clinical governance frameworks

What We’re Looking For

  • Registered Mental Health Nurse (RMN) with current professional registration

  • Experience working in community mental health settings

  • Ability to manage complex cases and risk effectively

  • Strong communication and multi-disciplinary working skills

  • Experience in supervision and mentoring (desirable)

  • Commitment to continuous professional development and quality improvement
